You may print out the following forms and bring them to the office with you. Otherwise, you may fill out the forms on-line so that Dr. Hegel may review the forms before your visit.

Otherwise we ask that you come 15 minutes prior to your appointment to complete the paper work. You will need to allow approximately one hour for your first visit. We look forward to meeting with you.
